COVID Student Vaccination Information
On Friday May 7th from 8 - 10:30am the health department will be set up in the High School cafeteria to administer the first Pfizer COVID -19 vaccination to Union County School Students ages 16 and over. The second shot will be administered at the cafeteria on May 28th from 8 - 10:30am. A parent or guardian of students ages 16 and 17 that want their child vaccinated must be present. Students 18 and over do not have to have their parents permission. All students will have to have the information and consent form already filled out and signed.  The fact sheet for the Pfizer vaccine will be emailed to your parents current email on file in Powerschool. You will also receive information on how to register for v-safe through vsafe.cdc.gov. V-safe is a program which you can participate in if you choose. It is designed to  monitor you after you receive the vaccine through text messaging from the CDC. This information will also be available on the High School WEB page.

When you arrive at the High School cafeteria on 5/7/21 and 5/28/21 you will need the following:
The information consent form filled out and signed.
Valid driver's license or an official picture ID.
Insurance card. (if you do not have insurance you will still be able to get the vaccine)
If you are ages 16 or 17, a parent or guardian needs to be present.
